When families are faced with multiple providers, recommendations, and decisions, the challenge is often not access to information. It is knowing how to bring everything together in a way that makes sense.
Families are left to carry the weight of important decisions without a clear, structured path forward.
Our Approach
We provide clarity and direction, grounded in empathy and real understanding of what families are navigating.
Our work focuses on helping families step back, understand what is happening, and move forward with intention.
This includes:

Why This Work Matters
Navigating multiple systems at once can feel overwhelming, especially when communication is fragmented, continuity is unclear, and access to the right resources feels out of reach.
Families are often expected to coordinate between providers, interpret recommendations, and make critical decisions without a clear framework.
The AFTER Organization was created to change that.
What We Do Differently
Many organizations provide resources, education, and advocacy.
Those things are important.
But for many families, the challenge is not access to information.
It is knowing what to do with it.
We focus on helping families take everything they have been given and turn it into a clear, structured pathway forward.
Founder
Christina Polanco, Founder & President
The AFTER Organization was founded by Christina Polanco, whose work is shaped by firsthand experience navigating complex medical, educational, and developmental systems.
She built this organization while actively navigating those systems herself, seeing in real time how overwhelming and unclear the process can be, even for someone with the background to understand it.
Christina recognized a gap that could not be ignored.
Families were being given large amounts of information, but very little clarity. Critical decisions were being placed in their hands without explanation, without structure, and without a clear path forward.
Because of her professional background, she was able to interpret, organize, and navigate what was in front of her.
But many families do not have that advantage.
She saw parents doing everything they could to advocate for their children, yet struggling not because they were incapable, but because no one had taken the time to truly explain the systems they were expected to navigate.
That gap is where AFTER was created.
Christina’s work focuses on bringing clarity, structure, and direction to families working through complex systems. She helps turn information into understanding, and understanding into confident, informed decisions.
Because families should not have to figure this out alone.
This work is rooted in both experience and a deep commitment to helping families find a clearer way forward.
